Local governments and state agencies use E-Pay to offer their constituents easier, faster payment choices. E-Pay participants can choose to accept credit cards, bankcards and/or electronic check payments online, by phone or in person.
Change of Address
If you have moved to a new address, it is imperative that you complete the "Change of Address" Form for your student in order to continue receiving District mailings as well as transportation information. Check here for our Required Documentation Guidelines for proof of address. Download and print both a "Required Documents" form as well as a "Request for Change of Address/Alternate Transportation" form.
Parents/guardians of high school and middle school students are required to fill out these forms, sign them, and submit them in person with the required proofs of address to the high school or middle school their student attends.
Parents/guardians of elementary students are also required to fill out these forms and submit them in person with the required proofs of address to the Student Assignment Center.
New Race/Ethnicity Reporting Categories
There are new state and federal guidelines for collecting race and ethnicity information. In the past, the State of Illinois required 6 categories, while the federal government recognized only 5. Starting July 1, 2010, both Illinois and the federal government will recognize the same 7 categories. All parents/guardians are required to fill out this form and return it to your school office.
